right in the heart. that size spear must have done massive damage. he probably stopped screaming and concentrated on trying to get his breathe so his damaged heart could try to pump blood to his brain. but since his heart was sliced so badly he could only survive on what oxygen was already in his brain. that would soon run out so he would lose conciousness and never wake up again
